Kindly complete the “Intake form” and “Child Background information” prior to this appointment. You are welcome to bring all relevant reports and/or referral letters along to this appointment. Please note that this appointment is specifically scheduled for parents/caregivers. We kindly ask that your child does not attend. The assessment appointment will be booked following the Intake Interview.

This appointment is for feedback on your child's assessment results. An hour is allocated for feedback. Should more time be required, the fee will be adjusted accordingly. Kindly note that this is a parent only appointment, but please use your child's name and surname to make the booking.
